Updated concussion/cardiac forms are available in the MAC MS main office. This form must be up to date and on-file prior to the beginning of practice. Please visit the MAC MS Main Office for the correct form CLICK HERE for web access. The form is page 8 of 9 in the physical packet.
All students playing a Fall Sport must have an Athletic Physical and Paperwork turned into the Athletic Department before they can take part in any kind of practice. Girls Golf practices officially start this Friday with all the other Fall Sports officially starting Monday, August 1st. Athletic Physicals and Paperwork can be printed off the website

Athletic All Sport Passes for the 2016-2017 season’s are available in the front office from Mrs. Greene. They can be purchased Monday – Friday from 9:00- 3:00.
