All athletes MUST have completed a TWO part process of online form AND IHSAA Health History Update and Consent/Release form on file with the MS Athletics Office prior to being allowed to participate in practice or competition in any way. Please Note: Physicals completed on or after April 1st, 2019 are being accepted for school year 2020-2021 athletic participation. A new physical is only required if a physical is not on file with MS Athletics Office for current 6th, 7th or 8th Grade student athletes. If your athlete was a 5th grader in school year 2019-2020, their hard copy physical was transferred from Mrs. Howay at MAC EL to Mrs. Hoffman in the MS Main Office. If your athlete was a 8th grader in school year 2019-2020, their hard copy physical was transferred from Mrs. Hoffman to Mrs. Greene in the HS Athletic Office.

This form includes the Code of Conduct, SCA (Sudden Cardiac Arrest), Concussion Information, Pay to Participate form, and Emergency Contacts. For the 2020-2021 school year, Part II forms will be submitted electronically only. High School Athletic Eligibility Form – Electronically submitted only. Electronic version must be submitted in the presence of student athlete(s) AND parent.

This form is to be signed by A PHYSICIAN(Nurse practitioners – NP – signatures are accepted). A parent and student also need to sign it. This regulation is made by IHSAA. This form is 4 pages in length.
